The attributes of Dementia are really more for the thieving and shady types, like Odds. The benefits of Mania are more inclined towards that of a mage. Being Charlie Manhuge, there's really no advantage.
Massive spoilers:
I think it all boils down to which one you want to beat to death. After becoming the dutch of dementia or mania, the dutch/duchess of the mania/dementia respectively defects to Order and becomes a priest.

not that much of a spoiler, but it's still a spoiler
until their scripted events of their deaths, which is later on in the story line -well, the one you haven't killed already- they are essential NPCs and cannot be killed by normal means\
