Sylvanas Windrunner from World of Warcraft. She rules the Forsaken, an undead race pretty much hell-bent on eradicating every living thing. Technically they're allied with the Horde, but Sylvanas likes to ignore orders and do things like turn civilians into goo, drop the Plague of Undeath on a populated city, and resurrect fallen solders to force them into her army. So, pretty evil I'd say. Oh, and she's an undead elf herself, so not human.

God, from the Silent Hill series. She is both the woman in a red dress and the monster on the right.
The goal of the omnipresent cult in Silent Hill is the rebirth of God, and through that, the beginning of the Apocalypse and the destruction of humanity. In scriptures, she is presented as strict and vengeful and, in later games, she is described as a malicious being. A character mention that you could either call her God or the Devil. All of her physical manifestations are violent and monstrous, with feminine features.

Sarah Kerrigan was a Ghost, a human with psyonic power. The Zerg detected this, captured her, and transformed her into the QoB. Her mind became a part of the Swarm, and she was under control of the Overmind. Sarah Kerrigan was not in control of the body at all.

Menardi from Golden Sun. In the original game she was the primary antagonist, and was only revealed to have good intentions (with questionable methods) in the sequel. She's from a clan of dragon people that are humanoid with some visible scales.

Barbariccia from Final Fantasy IV.
She is listed in the wiki as one of the Four Archfiends which are a group of powerful elemental beings and the strongest monsters in Golbez's army. Needless to say, she is evil.
