So, one hero has a magic spell that produces mushrooms, as well as huge clouds of smoke, and he gets killed while he can't see through the smoke?

well the mushroom part you have right but here is the explanation: Teemo is a champ that can lay mushrooms on the ground, singed is a champ that can make poison clouds (the purple stuff). whenever someone steps on a mushroom they get damaged and are slowed. Kennen (the guy in the purple) stepped on the mushroom and died.
